I am an out-of-network provider and do not contract directly with commercial insurance plans. Payment is due at the time of service.
Many plans offer out-of-network benefits. I can provide a monthly superbill — a detailed medical invoice — that you can submit to your insurance company for potential reimbursement. The amount reimbursed depends on your individual plan.
If you're unsure whether your plan offers out-of-network benefits, it's worth calling the member services number on your insurance card before we begin.
Your appointment time is reserved exclusively for you. If you need to cancel or reschedule, please provide at least 24 hours notice.
I understand that genuine emergencies happen and will use judgment in those situations.
You have the right to receive a Good Faith Estimate explaining how much your care will cost. Under the No Surprises Act, health care providers are required to give patients who don't have insurance — or who are not using insurance — a written estimate of the expected cost of services before treatment begins.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services, at least one business day before your appointment.
